Sliding Scale Discount Program
Making Mental Health Care More Affordable
At Vespertino Health, we understand that the cost of mental health care can be a barrier, especially for individuals who do not have insurance or are paying for services entirely out of pocket.
Our Sliding Scale Program is available to qualifying self-pay patients and may reduce the cost of eligible services based on household income and household size.
Our goal is to help make psychiatric care more financially accessible while maintaining high-quality, individualized treatment.
What is a Sliding Scale?
A sliding scale allows eligible self-pay patients to receive services at a reduced rate based on their financial circumstances.
The amount of the discount may vary depending on household income, household size, and the type of service being provided. Patients with lower household incomes may qualify for a greater reduction in fees, while patients with higher incomes may pay closer to Vespertino Health’s standard self-pay rates.
Eligibility must be verified before a sliding scale rate can be applied.
Who May Apply?
The Sliding Scale Program is intended for patients who are paying for their care directly and are not using insurance benefits for the services being provided.
You may apply if you:
-
Do not currently have health insurance
-
Are uninsured or temporarily without insurance coverage
-
Have insurance but are choosing not to use it for your care
-
Have coverage that does not include Vespertino Health services
-
Are otherwise responsible for paying the full cost of your visits out of pocket
Eligibility is based primarily on household income and household size and may be reviewed using current Federal Poverty Guidelines.
Approval is not automatic, and submitting an application does not guarantee a discounted rate.
Documentation We May Request
To determine eligibility, Vespertino Health may request documentation verifying your income, household size, or financial circumstances.
Examples may include:
-
Recent pay stubs
-
Most recent federal tax return
-
W-2 or 1099 forms
-
Social Security, disability, unemployment, or other benefit statements
-
Documentation of government assistance
-
Profit-and-loss statement for self-employed individuals
-
Recent bank statements when other income documentation is unavailable
-
Documentation verifying household size when needed
Additional documentation may be requested depending on your individual circumstances.
All information submitted for financial eligibility purposes will be handled confidentially.
Eligibility Review
Sliding scale eligibility may be reviewed periodically to ensure the discounted rate continues to reflect your financial circumstances.
You may be asked to provide updated income or household information:
-
Annually
-
When your financial circumstances significantly change
-
When your household size changes
-
When requested as part of an eligibility review
Failure to provide requested documentation may result in the patient returning to the standard self-pay rate.
Please notify Vespertino Health if your income, household size, or insurance status changes after you have been approved for the Sliding Scale Program.
A change in financial circumstances does not automatically mean that you will lose your discount. We may simply need to reassess your eligibility and determine the appropriate rate.
Vespertino Health evaluates sliding scale requests based on financial eligibility criteria.
Eligibility for the program is not determined based on race, color, national origin, age, sex, gender identity, sexual orientation, disability, religion, or other protected characteristics.
Ready to Apply?
If paying the standard self-pay rate is creating a financial hardship, we encourage you to submit a Sliding Scale Application.
Please complete the application below and provide accurate information regarding your household size and annual household income.
Once your information has been reviewed, our team will notify you regarding eligibility and any discounted rate that may be available.
Please note: Submitting an application does not guarantee approval. Sliding scale discounts are subject to eligibility requirements, documentation, program availability, and Vespertino Health policies.
